Health Services Information
School Health Clerk
Our school health clerk is our main caregiver in our health office. She is able to perform first aid and care for children who are not feeling well, perform health screenings, and manage immunizations and health records. This service is available from 7:30-3:30 everyday.
Our school nurse works with the health clerk and is at Meeker two days a week. If you would like to meet with Ms. Ashley Kendrick, our school nurse, please call the health office and leave a message for her.
Emergency Information
Please keep emergency contact information updated with the front office staff. We need to be able to contact you if your child is ill or injured.
Health Conditions
Please notify the health office for any changes to your students' health conditions. This includes new diagnoses, broken bones, surgery, etc. This allows us to properly and safely care for your student.
Medications
If you child must have medication of any type during school hours, including prescription or over-the-counter medicine, one of the following must happen:
- Parents/Guardian may come to school and give it to their child at the scheduled time
- Parents/Guardian and health care providers may complete and sign a "Medication Authorization" form. The medication must be in a pharmacy-labeled bottle or original packaging with instructions.
- Parents/Guardian may discuss with their health care provider an alternative schedule so the medication can be given outside of school hours.
Loaned Clothing
The health office has a limited supply of extra clothing for accidents. Please wash and return these clothes at your earliest convenience.
Immunizations
Students are not permitted to attend school without meeting state immunization requirements or having a provider signed medical or non-medical exemption form.
